OPD (Outpatient services)
The OPD stage records an outpatient visit for a service recipient who is already enrolled and has a master register entry.
Who sees the OPD stage
The master register determines which stages appear for a person. OPD is available for service recipients above five years of age; for children under five, outpatient illness is recorded through IMNCI rather than OPD.
If a service was provided at an age when it is no longer offered — for example immunization given at age two, viewed later at age seven — the earlier record still appears inside its own stage.
Steps
- Open the recording app and select the program and organisation unit.
- Find the person: use your saved working list (for example assigned to me, today) or search for the enrollment.
- Confirm the master register entry exists. If it does not, complete it first — see Enrollment and master register.
- Open the OPD stage for the current visit date.
- Record the visit details, including the diagnosis and the services provided.
- Add a note (Write Note) if something needs explaining for the next person who opens the record.
- If the person is being sent on to another staff member, set the Assigned user in the Assignee section.
- Select Complete to close the stage.
Points to watch
- Register before you record. Recording a service before the master register is complete can raise a validation warning. If you choose Save anyway, go back and complete the master register — otherwise the visit may not be counted correctly by age group.
- One visit, one stage entry. Use the correct visit date rather than editing an earlier visit.
- Referrals within the facility are handled by assigning the record to the colleague who will provide the service, not by re-registering the person.
Where the data goes
Completed OPD stages feed the routine monthly reports produced from the system (including HMIS 9.3 and 9.4). Data recorded accurately at the point of service is what makes those reports usable — see the Local Government section for how the reporting side works.
